Re-registration Form

If you are registering a new student , or have major changes, please complete both Registration Forms part I and II.
If your child is currently enrolled at Sacred Heart, please complete the Re-registration form.
Both the Registsration form and Re-registration form need to be returned to the office the a check for fees.
